To the Parents/Guardians of Ridgefield School District Students:
In accordance with Washington State Immunization Law (RCW 210.080) and Ridgefield School District Board Policy #3413, all parents/guardians must provide a completed and signed Certificate of Immunization Status (CIS) prior to a student’s first day of attendance. If a student does not have documentation for receipt of any or all of the required immunizations, he/she will be excluded from attending school, until proof is shown.
Washington State Law requires 1 dose Tdap vaccine for children attending 6th to 12th grade & age 11.
Beginning July 22, 2011, parents or guardians who want to exempt their child from immunization requirements must fill out and turn in the updated exemption form to their school. Parents must first talk to a licensed health care provider about the benefits and risks of immunization. The provider must sign the Certificate of Exemption verifying that the parent received this information or a provider may write and sign a letter that verifies the same information instead of signing the form. A health care provider does not need to sign the form for parents or guardians who show membership in a church or religious body that does not allow medical care from a health care provider.

1) Your doctor's office
2) Local pharmacy (Walgreens, Fred Meyer, Rite Aid etc.)
3) Sea Mar Community Health Center, call 360-896-5128 or 360-852-9070 for an appointment.
4) The Free Clinic of Southwest Washington - Walk-in Clinic available 1st, 2nd, and 3rd Wednesday of the month from 5:30-7:30. For more information, call 360-313-1390.
