2018-19 Open Enrollment begins April 9, 2018
\r\nUnion Ridge Elementary School Office is open Monday-Friday from 7:30AM to 3:30PM through June 29, 2018. Please call (360) 750-7600, if you have any questions.
\r\n\r\nHere is what you need to register your child:
\r\nKindergarten Enrollment Packet
\r\n1st through 4th Grade Enrollment Packet
\r\n-Verification of birth date - Child must be five years of age before September 1st to enter Kindergarten.
\r\n-Residency verification: 2 documents showing resident address & parent/guardian name
\r\n-Washington State Dept. of Health Required Vaccines for School Attendance
\r\nDiptheria, Tetanus and Pertussis: 4-5 doses DTaP, last dose must be given on or after 4th birthday
\r\nPOLIO: 3-4 doses, last dose must be given on or after 4th birthday
\r\nMMR: 2 doses, first dose on or after first birthday; second dose, 28 days or more after first dose
\r\nHEP B: 3 doses (must follow spacing requirements; please check with your health care provider)
\r\nVARICELLA (Chickenpox): 2 doses, first dose on or after first birthday; second dose, 28 days or more after first dose; or Health Care Provider verification of disease.
