  • 2 proofs of residency documents
  • Medically verified immunization records
  • Legal documents pertaining to child custody (if applicable)
  • Special programs documentation (HiCap/Gifted, IEP)
  • Foster Parents (School Notification Letter and Agency's Contact information)
